چکیده

Objective Processed electroencephalography (pEEG) is used to monitor depth-of-anesthesia during cardiopulmonary bypass (CPB). The SedLine device has been recently introduced for pEEG monitoring. However, the effect of hypothermia on its parameters CPB unknown. Accordingly, we aimed investigate temperature-induced changes in SedLine-derived CPB. Design Prospective observational study. Setting Cardiac surgery operating theatre. Participants 28 patients undergoing elective cardiac with Interventions We continuously measured patient state index (PSI), suppression ratio (SR), bilateral spectral edge frequency (SEF) and temperature. linear mixed modelling fixed random effects study interactions between core Measurements main results During maintenance, median temperature was 32.1°C [interquartile range (IQR): 29.8–33.6] at end cooling 32.8°C (IQR: 30.1–34.0) rewarming initiation. For each degree Celsius change PSI either decreased by 0.8 points [95% confidence interval (CI): 0.7–1.0; p < 0.001] or increased 0.7 (95% CI: 0.6–0.8; 0.001). SR 2.9 2.3–3.4); 0.001) 2.2 1.7–2.7; rewarming. Changes SEF were not related Conclusions hypothermic CPB, led concordant PSI. Clinicians using monitoring should be aware these when interpreting values.

Zinc in children undergoing cardiac surgery with cardiopulmonary bypass.

During cardiac surgery with extracorporeal circulation (ECC), a series of immunological and inflammatory alterations occur, which trigger oxidative stress1,2. Under the nonphysiological conditions during the ECC and due to the alterations related to ischemia-reperfusion, a large amount of free radicals is formed3. These are responsible for systemic inflammation and considerable structural and f...

Effects of hypothermia on thrombelastography in patients undergoing cardiopulmonary bypass.

Thrombelastography (TEG) correlates with postoperative chest drain output in patients undergoing cardiopulmonary bypass (CPB). In vitro incubation with heparinase allows TEG monitoring during CPB, despite heparin anticoagulation. Hypothermia impairs coagulation, but these effects cannot be assessed by standard coagulation tests performed at 37 degrees C. The aim of this study was to assess the ...

Electroencephalographic seizures during cardiopulmonary bypass.

Eleven cardiac operations are reported in which there was electroencephalographic and/or clinical evidence of seizure activity during cardiopulmonary bypass (CPB). In four patients seizure activity appeared after acute episodes of cerebral ischaemia resulting from either hypotension or pump-generated emboli occurring at the beginning of CPB, or from air embolism occurring at the end of CPB when...

Hyperglycemia during cardiopulmonary bypass is an independent risk factor for mortality in patients undergoing cardiac surgery.

BACKGROUND Hyperglycemia is commonly present in the perioperative period in patients undergoing cardiac surgery, even during administration of insulin. A direct relationship between postoperative hyperglycemia and mortality has been established in diabetic patients undergoing cardiac surgery. However, this relationship might be confounded because patients with poor outcome receive more glucogen...

Emergency Conversion from Off Pump to Cardiopulmonary Bypass in Patients with Coronary Artery Bypass Graft Surgery

Introduction: As off pump Coronary artery bypass grafting surgery (CABG) is a developing method in coronary cardiac surgery, most surgeons are anxious about the danger of emergency crash conversion. In this observational study we tried to show the rate and outcome of conversion . Material and methods: In this descriptive study about 477 coronary off-pump CABG patients were operated in Ghaem hos...
